Enroll an Incoming Kindergarten or New First Grade Student Never in Private Or Public School:
- Read the INCOMING KINDERGARTEN, FIRST GRADE ENROLLMENT REQUIRMENTS
- Complete the REGISTRATION FORM
- Each parent signs a CLA STATEMENT OF FAITH
- Become members of HSLDA-CLA needs number and renewal date
- REPORT OF HEALTH EXAMINATION FOR SCHOOL ENTRY completed by doctor
- Copy of IMMUNIZATION RECORD
- Copy of child’s BIRTH CERTIFICATE
- Complete the FACULTY INFORMATION FORM
- Complete the MEDICAL RELEASE FORM
- Complete the PROPOSED COURSE OF STUDY
- Complete the 2010-2011 Parent Agreement
- Submit enrollment fee
- Submit tuition
Enroll a New First Grade Student Transferring From a Private or Public School:
- Read the INCOMING KINDERGARTEN, FIRST GRADE ENROLLMENT REQUIRMENTS
- Complete the REGISTRATION FORM
- Each parent signs a CLA STATEMENT OF FAITH
- Become members of HSLDA, if not already
- Complete the PARENT AUTHORIZATION FOR RELEASE OF SCHOOL RECORDS
- Complete the FACULTY INFORMATION FORM
- Complete the MEDICAL RELEASE FORM
- Complete the PROPOSED COURSE OF STUDY
- Complete the 2010-2011 Parent Agreement(Parent Agreement)
- Submit the enrollment fee
- Submit tuition
