Enroll a New Student:
- Complete the REGISTRATION FORM
- Each parent signs a CLA STATEMENT OF FAITH
- Complete the (Parent Agreement)
- Become member of HSLDA-CLA NEEDS NUMBER AND RENEWAL DATE
- Complete the PARENT AUTHORIZATION FOR RELEASE OF SCHOOL RECORDS
- Complete the FACULTY INFORMATION FORM
- Complete the MEDICAL RELEASE FORM
- Complete the PROPOSED COURSE OF STUDY (First through eighth grade)
- Complete a COURSE STANDARD AND DESCRIPTION for each course (high school)
- Copy of personal immunization record - 7th grader
- Submit enrollment fee
- Submit tuition
