Re-enroll a student

• Complete the Re-Enrollment Form 2011-2012
• Update HSLDA membership-CLA will need number and renewal date
• Complete the PROPOSED COURSE OF STUDY (1ST-8TH)
• Complete COURSE STANDARD AND DESCRIPTION for each course (high school)
• Copy of personal immunization record-7th grader
• Submit Family Photo
• Submit enrollment fee
• Submit tuition
