Sports Registration
Christian Life Academy will offer CIF Sports for school year 2009-2010. Following are the sports offered for each season, registration period, registration fees and forms.

| The first practice for the spring season will be February 20, 2010. The regular season will end mid-May. | |||
A $75.00 late fee will be assessed if registering for a sport after the appropriate registration period. This late fee will not be assessed to students who are enrolling in CLA for the first time.
To Register For a Sport:
THE STUDENT/ATHLETE MUST BE FULLY ENROLLED IN CLA IN ORDER TO BEGIN THE SPORT REGISTRATION PROCESS.
- Complete and mail Sports Registration Form
- Submit one-half of the sports registration fee
Before the first practice:
- Submit the sports registration fee balance
- Submit the late fee, if applicable
- Preparticipation Physical Form (good for one year)
- Medical Release Form (for each season)
- Informed Consent Form (for each season)
- CIF Ethics in Sports Form (for each season)
