Sports Registration
Christian Life Academy will offer CIF Sports for school year 2010-2011. Following are the sports offered for each season, registration period, registration fees and forms.

| The first practice for football will be August 9, 2010. The regular season will end early November. The first practice for volleyball will be August 16, 2010. Playoffs will be held November 10-21. |

A $75.00 late fee will be assessed if registering for a sport after the appropriate registration period. This late fee will not be assessed to students who are enrolling in CLA for the first time.
To Register For a Sport:
THE STUDENT/ATHLETE MUST BE FULLY ENROLLED IN CLA IN ORDER TO BEGIN THE SPORT REGISTRATION PROCESS.
- Complete and mail Sports Registration Form
- Submit one-half of the sports registration fee
Before the first practice:
- Submit the sports registration fee balance
- Submit the late fee, if applicable
- Preparticipation Physical Form (good for one year)
- Medical Release Form (for each season)
- Informed Consent Form (for each season)
- CIF Ethics in Sports Form (for each season)
